How to Refer a Patient?

A Simple 4-Step Process

Step 1

Download and complete the genetic counseling referral form.

Step 2

Email the completed form along with any relevant medical records not already submitted to info@presciencedx.com.

Step 3

Our counseling team will contact the patient directly to schedule an appointment. If we are unable to reach the patient, we’ll notify the referring provider.

Step 4

After the session, a summary report is sent to the referring provider, including a consultation overview, care plan, and recommendations.
