Referring provider will download and fill out the genetic counseling referral form.
Providers sends the referral form, along with any relevant medical documents not already provided with the order, to info@presciencedx.com
The Prescience Diagnostics patient counseling team will contact the patient to schedule an appointment. If we cannot reach the patient, we will inform the ordering provider.
Following the appointment, a consultation overview is sent to the referring provider summarizing the discussion, plan, and recommendations.
